  • The immunoglobulins (Ig) are proteins used by the immune system to bind and neutralize foreign molecules entering the bloodstream. Also called antibodies, these molecules are composed of two pairs of disulfide-bonded heavy and light chains, each with a constant and a variable region. These pairs are, in turn, held together by two disulfide bonds between each heavy chain. The overall structure approximates a Y-shape conformation, where the variable regions are in the open ends. Together, these variable regions constitute the antigen binding site.There are five immunoglobulin isotypes (IgA, IgD, IgE, IgG, and IgM), depending on the type of heavy chains present. On the other hand, the light chains are of two types, kappa or lambda chains, which naturally occur at approximately 2:1 ratio, respectively. Mammalian immunoglobulin molecules always contain identical light chains, either both kappa or both lambda.Most of the light chains are attached to the heavy chains, but low and relatively constant levels of free light chains (FLC) are also normally present in healthy individuals. However, certain diseased states are manifested by abnormal levels of light chains, or over production of one particular type of light chain (kappa or lambda), which alters the kappa/lambda ratio. Physiological levels of bound and free lights chains can be monitored in serum and/or urine for diagnostic purposes, as well as for monitoring the efficacy of treatment regimens.
