  • The immunoglobulins (Ig) are the second most abundant proteins in human blood after albumin. These proteins are used by the immune system to bind to and neutralize foreign molecules entering the bloodstream. Of the five immunoglobulin isotypes (IgA, IgD, IgE, IgG, and IgM), immunoglobulin M (IgM) is the only isotype that is present in an immature B-cell. Upon maturation and activation, the cells begin to produce the other immunoglobulin isotypes. As a pentamer of identical subunits, IgM is the largest protein in the circulatory system and constitutes about 10% of all immunoglobulins in serum.The most important role of IgM undoubtedly lies in its ability to function early in the immune responses against blood-borne pathogens. As might be expected from its pentameric structure, IgM is very efficient in agglutinating particulate antigens and is particularly effective at complement activation. Since IgM antibodies appear early in the course of an infection, the levels of IgM in the bloodstream is useful in the diagnosis of recent infection. In addition, since IgM antibodies do not pass across the human placenta, its presence in a neonate's serum likely indicates intrauterine infection. Therefore, an accurate quantitation of IgM in plasma or serum is important for medical diagnostics, as well as for monitoring the efficacy of treatment regimens.
