It inhabits the mucus lining of the gastric epithelium. The majority of the colonizing bacteria are free living in the gastric mucosa, however a minor fraction (~1%) are attached to gastric epithelial cells. Even though H. pylori is generally believed to be exclusively extracellular some studies indicate that a diminutive fraction of the bacteria might enter epithelial cells. H. pylori is one of the most common pathogenic infections, occurring in an estimated half of all humans, with a higher prevalence of infection in developing countries than in the industrialized world. Colonization usually occurs early in life and is lifelong, unless treated with antibiotics. In a subset of individuals, the infection leads to serious gastroduodenal diseases such as peptic ulcer or gastric adenocarcinoma.
