NE issynthesized in the promyelocytes and promonocytes and is predominantly located in the primary (azurophilic) granules of neutrophilswhere it comprises a major component. Released from the neutrophil granules, NE acts as a potent proteolytic agent capable ofdegrading a wide range of proteins, including extracellular matrix proteins, clotting factors, immunoglobulins and complement. NE is animportant mediator in the killing of microorganisms and in the initiation of tissue injury in inflammation. NE is also considered to beimplicated in tissue destruction in pulmonary emphysema, glomerulonephritis, rheumatoid arthritis and adult respiratory distress syndrome(ARDS). Additionally it has been linked to tumour infiltration. The tissue-destructive properties of NE are counteracted by a varietyof endogenous inhibitors, including α-1-antitrypsin, human neutrophil elastase inhibitor (HEI) and elafin.
