  • The immunoglobulins (Ig) are the second most abundant proteins in human blood after albumin. These proteins are used by the immune system to bind to and neutralize foreign molecules entering the bloodstream. Of the five immunoglobulin isotypes (IgA, IgD, IgE, IgG, and IgM), immunoglobulin A (IgA) is the second most abundant form of the total immunoglobulins in serum (1-3 mg/ml), being surpassed only by IgG (6-18 mg/ml). However, secretory IgA (sIgA) is the most abundant immunoglobulin in colostrum, milk and other secretory fluids. The relative abundance of the different isotypes is generally constant in mature adults. Changes in the relative abundance of the different isotypes usually indicate immunological disorders or disease states.IgA is considered the most enigmatic immunoglobulin isotype, but well known biological functions include modulation of inflammatory responses, inhibition of the mobilization of phagocytes, and mediation of the humoral immune defense at the mucosal level (mucosal homeostasis). For example, individuals with selective IgA deficiency, the most common form of immunodeficiency, have an inherited inability to produce IgA, while able to produce normal levels of IgG and IgM. These individuals lack immune defenses in their mucus membranes and are susceptible to infection. Therefore, an accurate quantitation of IgA in plasma, serum, and other secretory fluids is important not only for medical diagnostics, but also for monitoring the efficacy of treatment regimens.
