  • Cytomegalovirus (CMV) belongs to the Herpes virus group, which also includes Herpes Simplex virus type 1 and 2 (HSV), Varicella Zoster virus (VZV) and Epstein-Barr virus (EBV). A common characteristic of these viruses is that they can cause latent infections. The majority of individuals are infected with CMV at some time during their lives. In normal adults and children infection, reinfection or reactivation of CMV usually is asymptomatic. However, serious illness can occur in cancer and immunosuppressed patients or as a result of intrauterine or perinatal infections. Blood transfusion and organ transplantation are important causes of CMV infections in immunosuppressed patients. Transmission of CMV to organ recipients can be prevented by selection of donors which are seronegative for antibodies against CMV. Another important cause of morbidity in immune suppressed patients is reactivation of endogenous latent virus due to immunosuppressive therapy. Serological diagnosis of CMV infections can be performed by testing for a significant rise in CMV-specific IgG antibodies in paired sera and testing for CMV-specific IgM and IgA antibodies. It is advised to perform a combination of IgG, IgM and IgA testing. Significant rises in IgG antibody titers cannot always be found, depending on the time of serum collection. In primary CMV infections detection of a CMV-specific IgM and IgA response is the best diagnostic criterion. For recurrent CMV infections detection of CMV-specific IgA is the best diagnostic criterion.
